• Clinical evaluation and discussion of treatment options

This membership includes ongoing provider oversight and management for patients receiving peptide therapy through Elevare Wellness, along with the cost of prescribed peptide medications.

Anti-Aging & Longevity Support
This membership provides ongoing medical management for women pursuing bioidentical hormone replacement therapy (BHRT).
Services include provider oversight, hormone management, dose adjustments as clinically appropriate, and follow-up care related to hormone therapy.
Membership includes scheduled medical follow-up visits every 3–4 months via telehealth, along with asynchronous messaging related to hormone therapy management.
Medications, laboratory testing, and supplements are not included and may be prescribed or recommended based on individual needs. Medications may be obtained through insurance or paid directly through a pharmacy of the patient’s choice.
This program does not guarantee specific outcomes and is intended to support symptom management and overall health when clinically appropriate.
Women’s Hormone Optimization
Women’s Bioidentical Hormone Replacement Therapy Membership (Medications Included)
Starting at $199 / month
This membership includes all services outlined in the Women’s Bioidentical Hormone Replacement Therapy Membership (Medications Not Included), including ongoing provider oversight, hormone management, scheduled medical follow-up visits every 3–4 months, and asynchronous messaging related to hormone therapy care.
In addition, this membership includes prescribed bioidentical hormone medications, with pharmacy coordination and shipment through third-party pharmacies, including compounding pharmacies.
Starting pricing generally applies to single-hormone or lower-dose protocols. Combination therapy, higher dosing, or more complex treatment plans may fall within higher pricing ranges. Final monthly pricing is determined after medical evaluation.
Laboratory testing is not included and may be recommended based on individual needs.

Women’s Hormone Optimization
Women’s Hormone Support Plus (Optional Add-On) +$99 / month
This optional add-on provides additional lifestyle and symptom-support services for patients enrolled in a Women’s Bioidentical Hormone Replacement Therapy Membership.
Services may include hormone-related education, cycle and symptom tracking guidance, sleep and stress support, and lifestyle counseling relevant to hormone balance.
Hormone Support Plus includes structured accountability and check-ins, which are primarily asynchronous. Brief scheduled check-ins, when needed, are limited to approximately 15 minutes.
This add-on does not include medications, laboratory testing, or custom nutrition or exercise programming and is intended to complement — not replace — medical care.
