Introductory Wellness Consult
This visit is for you if you:

• No medical advice, diagnosis, labs, or prescriptions are provided
• If you move forward, the consult fee will be credited toward the Initial Comprehensive Evaluation when scheduled within 14 days

Initial Comprehensive Evaluation
This visit is for you if you:
• Are ready to begin care
• Want a full medical evaluation
• Are seeking treatment planning for weight loss, hormones, or wellness optimization
What this visit includes:
• Comprehensive medical review
• Laboratory review and interpretation
• Risk assessment and contraindication screening
• Individualized treatment plan
• Determination of candidacy for therapies such as semaglutide or tirzepatide
Important to know:
• This is a medical visit
• Intake forms and consents are required prior to the appointment
• Labs are not included in the visit fee
The evaluation fee is applied toward enrollment in a 12-Week Foundation Program.
WEIGHT OPTIMIZATION MEMBERSHIPS
Designed to establish safe dose titration, metabolic stabilization, and measurable clinical progress.
All Foundation Programs include:
• Prescription medication for 12 weeks (when clinically indicated)
• Structured dose escalation
• Pharmacy coordination and direct shipping
• One scheduled follow-up visit (Week 6)
• Secure messaging related to treatment
• Nutrition and protein education materials
• Progress tracking tools
May include prescription semaglutide therapy when clinically appropriate.

• Prefer to pay their pharmacy directly
• Are not utilizing bundled medication programs
Includes:
• Ongoing provider oversight
• Prescription management (medications not included)
• Treatment plan adjustments
• Follow-up visits every 8–12 weeks (telehealth, 30 minutes)
• Secure messaging related to care
Laboratory testing and medications are billed separately.

• Protein and calorie target education
• General resistance training recommendations
• Habit and lifestyle coaching
• Bi-weekly asynchronous accountability check-ins
Does not include:

Peptide Therapy Membership
Starting pricing generally applies to single-peptide, lower-dose protocols. Monthly pricing may vary based on the specific peptide(s) selected, dosing requirements, or use of combination or advanced protocols.
Laboratory testing is not included and may be recommended based on individual needs.
This membership does not guarantee specific outcomes and is intended to support overall wellness goals as part of a personalized care plan
Women’s Bioidentical Hormone Replacement Therapy Membership
This membership provides ongoing medical management for women pursuing bioidentical hormone replacement therapy (BHRT).
Services include provider oversight, hormone management, dose adjustments as clinically appropriate, and follow-up care related to hormone therapy.
Membership includes scheduled medical follow-up visits every 3–4 months via telehealth, along with asynchronous messaging related to hormone therapy management.
Women’s Bioidentical Hormone Replacement Therapy Membership
This membership includes all services outlined in the Women’s Bioidentical Hormone Replacement Therapy Membership, including ongoing provider oversight, hormone management, scheduled medical follow-up visits every 3–4 months, and asynchronous messaging related to hormone therapy care.
In addition, this membership includes prescribed bioidentical hormone medications, with pharmacy coordination and shipment through third-party pharmacies, including compounding pharmacies.
Laboratory testing is not included and may be recommended based on individual needs.
